While the Internet community derides paper as "dead trees", no business can function without paper work orders, invoices, and the other accoutrements of every brick-and-mortar business. This page describes various forms we have created.

Providet is one of the larger tri-state suppliers of commercial and residential services in the areas of duct cleaning, kitchen vent hood cleaning, air filter installation and maintenance, and similar services.

As part of updating its printed materials, Providet wanted new carbonless business forms. (Kuru also created new business cards, stickers, and stationery.)

This work order is a carbonless form with two components: a top sheet remaining with the customer and a bottom sheet retained by the company. The company copy has additional fields, such as billing information and a repeat visit explanation, which are not present on the customer's copy.

The redesigned form has more information than the original, yet it fits in the same 8.5 x 11.0 inch footprint. We changed the layout from portrait to landscape to better utilize the available space.

The original form is shown below. In addition to being crowded and in a difficult-to-read light red ink, the checklist moves from right to left and row to row, which is counter-intuitive.
